Admission will be granted on the basis of entrance test and an interview. The candidates will be selected strictly on the basis of merit. Candidates have to apply on the prescribed application from. Only completed application form along with all the attested photo copies of the testimonials have to be submitted to Principal of Laxmi Nursing Training Institute at Sarai- Kazi / City office, Varanasi. Selected candidates will have to deposit the prescribed fees on or before the last date according to the prescribed fee structure. Along with application form attested copies of followings must be attached.

1. a. High School mark-sheet
2. b. High School Board Certificate
3. c. Higher Secondary (10+2) or equivalent mark-sheet
4. d. Higher secondary certificate
5. e. Character Certificate from school last attended/Gazetted Officer
6. f. Transfer certificate from last school attended. The fee once deposited will not be refunded and the caution money deposited at the time of admission will be refunded only on completion of course.
